Blue screen: APC INDEX MISMATCH

Can't get steamVR to work. Every time I try and start it up, I get a blue screen saying APC INDEX MISMATCH. Everything worked fine before and I haven't installed any new drivers or software lately.

Things I've tried.
1. MSCONFIG - disabling all program and services except for microsoft services. Still blue screens when I start SteamVR.
2. Unplugging all USB except my keyboard and mouse.
3. unplugging and plugging in the vr headset. Doesn't seem to work either.
4. Uninstalling and reinstalling steamVR. Doesn't work.

Is the latest version of steamVR just really buggy or something? Cause I don't think anything else could cause such a strange blue screen and it only happens when I open SteamVR.

Remove excess USB devices. Could be keyboard, mouse, bluetooth USB dongle.
I would get a crash from a logitech device and a firmware update to that logitech device fixed my issue.

It's worth a shot.

I tried an older nvidia driver too. That didn't help.

I just have a Microsoft Natural Ergonomic 4000 Wired Keyboard and a razer mouse attached right now. I'll look for firmware updates right now, but the thing is I have been using VR fine for a long time with both of these and was even playing last night and suddenly today all I get is that blue screen and occasionally SteamVR doesn't even detect my headset.

I'm also noticing a lot of people having issues since the last update. I wish I could try an older version of steamVR. Then I would know if it's that or not.

So I installed SteamVR beta just now and now everything works fine, but as soon as I go back to the non-beta release I get blue screens. So it looks like the current version of Steam VR has major bugs.
