Valve Index controller has no tracking
My left controller has no tracking whatsoever. It started about 5 minutes after upgrading the firmware to the latest, and about 20 minutes after I turned it on for the first time. Is there any confirmed way to rollback or reset firmware? The reset instructions on their website doesn't reset the firmware (at least that I can tell) and there's no clear indication on exactly how to reset firmware, or even tell if it's a hardware issue. Is anyone else experiencing the same problem?

(EDIT: I've done tons of things to try to get this to work, even going so far as to try and use a different PC with completely different base stations, and yet no luck. I feel almost completely stumped.)

In the end i got an RMA.
Haven't had any problems sinds then yet. I would also request an RMA if i were you.

My left controller just stopped tracking. Same symptoms everyone else has described. I just got the full kit two days ago (Mar 6 2021), so these bum controllers are still available.

Tried the tapping trick after un/reinstalling steamvr. Left controller is working again. Better than it did before. We'll see for how long.

Ok, after hours of researching, I thought: well maybe this has to do with the tracking on the base stations. After searching for a way to reset them I found a way to manually reinstall the firmware for the headset.

-Disconnect Index headset from adapter
-Restart computer (do not open SteamVR *additionally I uninstalled SteamVR, but am not sure if this was part of the solution or not*)
-Hold button on bottom of headset
-While holding button above, plug headset back into adapter. (This will place the headset into what some are calling here: https://www.reddit.com/r/ValveIndex/comments/c8wvft/how_to_reset_hmd_back_to_factory_firmware/ a bootloader mode.)
*You should see red blinking lights on the front panel of your headset. If not, start over
-Open SteamVR (reinstall if you have uninstalled it)
-SteamVR will prompt you to update device
-Re-pair controllers

Viola, these steps solved my issue perfectly. Will follow-up if issue persists after this.

i started having the same problem as well on my left controller was going to play beat saber when i logged in to steamvr and noticed my left hand gone and it wasn't tracking I can still feel vibrations and inputs but nothing has worked when i try to fix it I've tried almost everything that steam support has told me I'm glad to at least know I'm not the only one that has this problem

I tried this and it prompted to do the updates to the headset but the left controller still doesnt track im going to RMA this if I can

Not joking, slapping my controller repeatedly fixed the issue. Tracking was completely dead out of nowhere and after slapping the controller it works again.

One more reason to tell that to the support and get a normally working device. :p
Do you want working or partially working hardware is the question here... I prefer my very expensive hardware to work properly all the time. :P

I fixed this by contacting support.

For my particular setup and firmware versions and initial troubleshooting, this is what they recommended:
Please try performing a 'hard reset' on your controller. To do this:
- Plug the controller in via USB and leave it to charge for at least 10 minutes.
- Unplug it and hold down the System Button for 15 seconds, then release.
- Power the controller on again.

Please attempt this a few times. The controller should recover.

My room next day I turned on the other base stations for playing room scale games the third base stations isn’t on that day my right controller lost tracking Verry annoying and the next day started playing and now my left controller is losing tracking but before it was perfect all started after some base station update after turning on the third base station for the first time

I finally resolved my problem with tracking. The controller are OK. It seems a combination of poorly written index software and hardware. I guess that when doing tracking the software handles the controllers in the order of their pairing so that when the hardware is sluggish only the first controller gets enough traffic to be tracked. If they changed the order periodically both controllers would have tracking and they could detect the hardware problem and give me a warning to check the hardware.

In my case the hardware was the Eltron EJ168 USB 3.0 controller. Changing USB ports did not work for the obvious reason that the motherboard had only one controller that handled all ports.

So I took an old dusty PCI-e 4 x USB 3.0 card with another controller (Fresco Logic). I had also to buy a PCI-e riser because the only free PC-e slot was covered by my graphics card. Risers are inexpensive but some are of poor quality, read reports before buying. Choosing PCI-e USB card verify that it has a different chip than you already have.

I connected the index to the card's USB port and that did the trick. Both index controllers are tracked now.

I had the same problem, I moved the headset usb cable to a different usb hub (usb 2.0 instead of 3.0 even) and all started to work, will go for your option as well to install a fresh usb 3.0 card

I just got the HTC Vive Pro2 Headset and the Valve Index controller, I followed all the instructions but was having difficulty with the Controllers staying paired(wirelessly) I was only able to keep them going while plugged in using the(type-c charging cable)

I noticed the controllers on SteamVR kept saying "Searching" I was able to resolve this issue by switching the headset USB from USB 3.0 to USB 2.0 and the controllers stayed connected. VERY HELPFUL POST. Thank you very much!

The way I fixed it, is I just started smashing the controller in all direction with different forces while the base station was searching for it, and it worked! I'm not sure this fix is gonna last tho x3

Here is how I fixed my non tracking controller but it requires you to have full body trackers (or you to buy dongle).
-Turn off all the controllers and trackers
-Disconnect headset cables from your PC but leave steamvr running
-Make sure your dongles are connected and pair non tracking controller normally (it will connect to dongle instead of headset)
-You can either connect second controller to dongle also or reconnect headset so it will be connected to headset
-Connect trackers if you are using them (some will be connected to dongles and some to headset)
