Valve Index controller has no tracking
My left controller has no tracking whatsoever. It started about 5 minutes after upgrading the firmware to the latest, and about 20 minutes after I turned it on for the first time. Is there any confirmed way to rollback or reset firmware? The reset instructions on their website doesn't reset the firmware (at least that I can tell) and there's no clear indication on exactly how to reset firmware, or even tell if it's a hardware issue. Is anyone else experiencing the same problem?

(EDIT: I've done tons of things to try to get this to work, even going so far as to try and use a different PC with completely different base stations, and yet no luck. I feel almost completely stumped.)

What color is the LED light on the controller when it stops tracking? Do inputs on the controller continue to work? For example, can you open and close the dashboard by pressing the system button on the left controller?

If you can share a SteamVR system report, that will help diagnose the issue. Start SteamVR and turn on the controllers, verify that you're still seeing the issue, then right click the SteamVR window with the device icons, select Create System Report, then Save to File. You can link it here through Google drive, pastebin, or some other text file sharing service.

The controller LED is green. The controller can still send inputs just fine. In SteamVR, the controller icon is green, yet is flashing. Here's the link to the SteamVR system report: https://drive.google.com/open?id=1so3PRQ9STWHC1XGptZU2RL1uI1VRv5O_

I have this exact same issue only it's my right controller. I received the controllers and immediately updated both of them. My left controller is tracking just fine but my right doesn't. The light on the controller is green but in SteamVR it's flashing green and says it's not tracking.

Here is my system report: https://drive.google.com/open?id=11Hq-9dstzA30tvDe13JO8HfFCAeKuvLO

EDIT: I made this system report before updating my base stations. I've since updated them and the problem is still there

I'm having the exact same issue. At first it was my left controller, it pairs perfectly fine and it can send input to SteamVR but the icon is flashing and it has no tracking. I've reinstalled SteamVR twice and updated to the beta branch, and at some point along the way the problem switched to my right controller. Now my left controller pairs and tracks great but my right controller will only pair and send inputs without tracking.

Fireheart, would you mind posting a system report also?
Fireheart Jun 28, 2019 @ 2:00pm
Yeah here it is. Right now SteamVR won't start without an error telling me to restart but I grabbed the report anyway, if that makes any difference.

It randomly stopped for me after a while, but it I find a permanent solution I'll come back to this thread.

Please do, haven't been able to use my Index yet. I'm still messing with some old Vive and Oculus solutions to see if anything helps. I did find out that the controller tracks perfectly fine when connected via USB, but as soon as it gets unplugged it loses the ability to track.

Because it was brought up I should note my controller does not track when plugged in.

This happened to me as well.
I did put a support ticket in yesterday, and for the moment I'm leaving it open just in case the issue returns, however their response says it could be from one of the trackers not functioning properly inside of the controller. I'm really hoping it's not that.

I'm having the same issue with my right controller. Before starting anything, performed firmware update on all devices. At first everything was fine, then the right controller lost tracking completely.

Tried everything on the troubleshooting page and no go.

If anyone gets some sort of solution, please post the results.

I have a ticket open, but not expecting a response for the weekend.

I'm having the same issue too, also the left controller still isn't detected even when plugged in

I am also having the same issue with the left controller. The inputs seem to work fine but it is not tracked by SteamVR. Below is my SteamVR system report

Still no solution here. Don't know about you guys but I still haven't been able to use my headset.

I think this is either a firmware or hardware problem. I'm using the index controllers with my OG Vive setup and both of my Vive wands still work just fine.

I opened a ticket yesterday and to my surprise I actually got a response today (on saturday). No magic fix yet though.
