should i play with this setting turned on or off to improve FPS as its kinda struggling

This setting has nothing to with increasing fps, it locks your framerate limit to you monitor's Hz, and you would only use it if you really wanted to decrease screen tearing. I suggest you keep it off to remove frame limit.

well i can play at a decent FPS but sometimes drop down to 40 and have random lag spikes but after a while the game becomes completely unplayable

This almost certainly wouldn't be helped/hurt by Vsync, although there have been a few games here and there over the years that have had major performance hits with Vsync on

Ultimately, Vsync on or off shouldn't impact anything, but it's worth giving a try turning it on/off and playing for a while. It can't hurt to try different game settings to try and see if you can figure out what the problem is although degrading performance over time suggests another problem

check your thermals first off. cpu mainly if its overheating i.e anything over 90 Celsius in a game is hot, you might wanna look into undervolting.
i had the same problem a few months back and i found that to be the cause and under-clocking the cpu slightly and undervolting it a little too helped in reducing thermal throttling which in turn gave a more stable fps.
you could also try rivatuner to hook the games fps.
my recommendation is to find your games average fps in ideal scenarios and take off about 20-30 frames and after adding the game to rivatuner setting it to that value.
for instance i can achieve 140 fps in the open country side but moving into a city itll drop to 100 or sometimes 90 sometimes even 80 if theres people.
so in rivatuner ill set the fps limit to 110 ; that way when i do get frame drops its not as dramatic and doesn't cause stutter in the frame animations.
if you download msi afterburner you get rivatuner alongside it; both are great for controlling your fps and getting an accurate read on your hardware.
so yeah, check your cpu thermals, and er use rivatuner to cap your fps because vsync sucks :D

I play DayZ with it off. Having it off. In my case gives 100+ FPS. Try it and find out.

The thing is, if you don't have a monitor that supports 100+ FPS, then you're not seeing any of it

Vsync matches the frame rate of the game to your monitor's refresh rate to prevent screen tearing, so anything more can cause such problems. From my recent experiences, I'd suggest playing with Vsync on if your frame rate is above your monitor's refresh rate (59hz, 60hz, 120hz, 144hz, etc) unless it causes your game performance to become worse

I can't say I've ever had any screen tearing issues with 120/144hz either and it's most likely due to the type of connector used and/or the quality level of said connector. In my case, the only monitors that have ever shown screen tearing were older monitors using older gen HDMI technology that only supported 1080p/60hz whereas I have never see screen tearing on my Displayport monitors. I haven't had access to nor used any sort of newer HDMI monitors that use HDMI standards used in 4K TVs, so I can't say how much better it would be, although it's almost certainly significant

I think it's worth mentioning that vsync introduces input lag too. Aim might not feel as sharp and snappy with it on. The difference isn't really noticeable to a casual stroller, but for more competitive gamers it is.

huha you folks are morons. if you are experiencing problems that gradually get worse to the point of the"games unplayable" then its obviously is not a framerate issue. . . once more i would advise you to check your thermals. you probably have your cpu overclocked to the point its cooking.
you dont need to have overclocked manually to reach this effect; simply having turbo boost and ♥♥♥♥ like that enabled CAN DO THAT TO YOUR CPU.
i bet your cpu is cooking at about 100 C while your playing ; so you get half an hour to hour in game and it turns itno ♥♥♥♥♥♥♥♥♥♥ because 30 mins to an hour is how long your cpus been cooking for. . . so once more, check your thermals and if its running above 90 C on the cpu. . do a little undervolt on intel xtu or whatever the equivalent is for amd cpus and continue to monitor thermals until an ideal temp/power balance is achieved
