How can you tell a player is friendly or not?

There’s no answer to this. You know how many times I’ve died to someone minutes after they told me they were friendly?

Don’t trust anyone until you’re running together for 20-30 mins. And even then it’s not guaranteed safe.

It may sound rude, but if they feel like you are new or are not 'offering anything' substantial in terms of skills that would benefit them, they may just KOS. It's always going to be a gamble but talking your way out of situations is a good course of action, sometimes.

True Day Z old school is "Q" & "E" lean left lean right.Or immediate confirmation "yes" to: "are you friendly" question. Mostly solo player,groups(squads) will kill you mostly.

I personally will consider you to be hostile and will kill

in my experience most players that have invested time into their gear are sketchy. trigger finger from the get go. I am the same. that is why i play solo(mostly). But if you run into a fresh player and you are fresh as well likely you can spark a convo and develop a long standing friend in the game.
Most of the friends that i have played dayz with in the past have been met in game when i was stuck with my pants down. LOL

Not really like real life. The majority won't KoS and the majority won't backstab you like in game. Groups that are split up across the map won't be able to do coordinate their actions..
Why? Because people are not psychopaths and second, people can't use voice programs to communicate ( metagaming ) and there are no working radios in game.

Man i wish there was a little device that you could use to communicate with eachother.... Something like a radio station... but alot smaller... ...you know... something to walk and talk...
