Report a translation problem
In game VOIP has a lot of game specific factors that affects it's output and quality that has nothing to do with the actual VOIP quality.
Examples:
Distance greatly affects VOIP volume and falls off at a relatively short distance.
Helmets make VOIP sound muffled for the speaker.
Gas masks also impact VOIP volume and also sound muffled.
Yeah I rarely use in game VOIP with the exception of encountering people outside your group, then you really have no choice.
EDIT:
IIRC, sebi1106 below jogged my memory, there is a server side setting that controls VOIP quality, the servers you were one were probably set to low. I am too lazy to go and verify settings on our server but VOIP sounds fine for us.

vonCodecQuality = 20; // Voice over network codec quality, the higher the better (values 0-30)
It all depends on the server your playing on.
When everyone sounded awful, the server probably had poor VoIP quality.
To be fair in the thousands of hours of playing I don't think I need dolby surround to listen to people scream NOOB and 1337 and GIT GUD and then cuss you when you kill them in any higher quality. :)

Or, more common... A device that TELLS them to do it.
it works fine for most people, its got to be a setting issue for you guys or bad sound card and mic issue etc
