Most likely you have an infected wound.
I REALLY (times 100^100 * infinity) wish they'd implement different sickness icons for different ailments.
I get the whole 'listen to your body feedback' and 'monitor your actions' play style but really...I am pretty sure in a survival world I would be able to tell the difference of being sick from eating bad food and an infected cut.
Good news is you get used to what causes what after a bit or you just spam vitamins.
Keep food and water maxed out helps also but I think that was tweaked a little in 1.19 - I'd have to go read the notes again...or was it in 1.18?

The first stage is treat with iodine/alcohol tincture/disinfectant. One application to your body will do it, or disinfect your rags before use. Tetracycline does not work in this stage.
At stage two the grunting in pain sound changes. From this point you must use tetracycline to cure yourself. Take one tab directly after the last one ends and take one more after the disease symbol disappears.
You spawn with two bandages. Your first priority should be to get a knife so you will immediately have access to rags. Almost all spawns are near a civilian town where not only will you find alcohol but also iodine and disinfectant spray that all serve the same purpose to give you clean rags to bind your wounds with.
