It seems like this can happen in a number of ways, though the descriptions don't quite match yours. The comments are about finding either just the Regent, or both him and the torturer dead in various places.
Apparently so, but if you look at the link I attached, it seems that that isn't always the case.
Personally, I couldn't say one way or the other, since it's never happened to me. One person in that link stated categorically that it happened after trying it several ways. The only way they could avoid the death was by 'exiting that map before the guards escorted him to the lobby.'
So, that player went with the non-lethal option.
Since a number of players have had this, it's looking like a bug/glitch to me. Nobody seems to know what triggers it, though.
Then there's the chaos mechanism that could play a part.
I never had it, but I always played low chaos... very low. I like stealthy games! I honestly can't recall how I played this... it's been years. I'd guess that I scuttled around keeping out of sight. It'll be interesting when I get that far this time. I've just decided to dabble with Dishonored when I get fed up with Styx: Master Of Shadows... we'll see.
Could be why I never triggered it. I was sometimes surprised by the 'bodies detected' stat, or something. I always tried to hide them carefully, if I knocked any out. Mostly, I tried not to even KO them.
oddly enough despite those 2 dying i actually still got no hostiles killed. i guess technically i didnt cause the death so the game doesnt count it.
... on going back up, Dark Vision showed a guard rushing out from the main entrance. I ran to find cover, but he went down past the zappy thing, opened the door to the balcony and found the body, then ran around screaming that he was going to find me!
I reloaded a save because I didn't want alarms or detections, did the same thing again and nothing bad happened. There's no way that guard could see me carrying the body, so why the first time?
This level seems a bit buggy, to me.
