"Capable of subduing the flame of frenzy if inherited, allowing one to cheat fate and avoid becoming Lord of Frenzied Flame" - taken right from the item's description.
going frenzy flame into needle, is also the only way to save melina and still get a happy ending. its actually weird that there is no way to capitalize on that, like a melina ending
(or that none has been found yet)
i dont want to believe that nobody has gone save melina with frenzy flame into ranni ending yet (just mentioned it as ranni is the most obvious connection to melina)
If you want the Frenzied Ending don't use it at all. It's not needed for any of the actual endings unless you want out of the Frenzied Ending.
My random conspiracy theory is that Melina's tied to some outer god or another and the needle blocks her just like it does the Three Fingers. It's not just for the Flames after all, it was made for the God of Rot and it explicitly works on all of the outer gods. If Melina's connected to one of them, it makes sense that it'd block her from talking to you. It'd neatly explain why she isn't in the other endings, since the only way she could be alive is if you used the needle.
I think the common theory is her being somehow connected to Destined Death due to her eye being similar to Ranni's.
