Killed instantly by dismounting
Flew into a cave near the green ark with lots of crystal in it. Dismounted and died instantly and the game told me I killed myself. No way to get my (first) tame back. Lost basically all progress since joining the map.
I have no problem dying through my own stupidity, but there was no way for anyone to know you would die there. Absolutely zero indication.
I will not play on this map again and will advise anyone to do the same. Dying randomly, without the ability to know of any danger is tremendously anti-fun and unfair.

Are you referring to the cave through the waterfall?

I went and tested this out and found the "cave with lots of crystal in it" on both my singleplayer and on my dedicated server I host. Was in fact the cave through the waterfall. I also had a few other members on my server test this. We were unable to replicate the issue what so ever. We tried unmounting a few different kind of dinos as well to see if it was dino specific. Tried a raptor, baryonyx (barely fit through the cave entrance), and a friend comically tried on one of his doeds.

Are you running the map in singleplayer, on a dedicated? Are you running mods?

from my understanding of your comment and how ark do some stuff...
it was probbably not even the map it was ark itself that killed you with the anti meshing mechanic

I'm with Flockerkill on this one. It was likely your game. Members of my server have taken an interest in the cave since I mentioned this possible issue. The issue still hasn't been replicated. Ark is unpredictable with its engine bugs, to a point I don't even think Wildcard knows how to resolve all of them. The engine has drifted quite a bit from the original UE4 engine since Wildcard started making their own patches.
