What do you mean doesnt start? Are you crashing or does it appear " frozen ", in the case of frozen, thats normal because its loading.
i have same exact problem, 100% processor usage (i have i9 12900K).The loading screen starts, then the small logo at bottom right corner spins few times and it stops. at this point everything is froze until i close the process from system level.

It can be, yes. Can be 3 mins, can be 60+ mins. Depends on your pc specs and cpu.

Is this only the first time it loads up or will it be the same everytime?

First time because it has to cache TOR shaders.

Mine just outright crashes the moment it changes from the vanilla loading screen to the modern one

Is your load order correct ? Meaning harmony on top, then all TW modules, and as last the 3 TOR modules? Having them out of order will crash.
