Maybe if Mod Config Menu support gets added, you could do something similar to how the Sewage Collection and Golden Collection mods allow you to turn on/off Reflection/Fiche natural spawns respectively. You could also go the Loot Deck route and be able to directly tweak the actual spawn rates of various modded pickups, that be chests, cards, or tainted hearts.

In TBOI:Four Souls Requim,T-Isaac Starts with Spindown Dice,& T-Lost starts with Soul Bond,and I'm thinking they can be toggled via the Mod Config Menu while Rep. Plus is enabled.
Ultra:Range up. When the player dies,It will now give the player a smelted golden Brain Worm trinket!
Ultra:Bigger creep
When it kills an enemy,it’ll make a fire wave!
Ultra:When it kills an enemy next to another one,it will charge to that instead of disappearing.
Ultra:Upon reviving the player,it will give him a holy shield!
Both states:Contact DMG up
Fighting state:Bigger creep,shoots 2 teeth shots simultaniously.
Ultra:
Both states:Orbits closer to the player
Fighting state:Teeth shots have a chance to be dead teeth
Ultra:Enemies affected by Cherry farts will fart also,spreading the effect! They no longer drop half red hearts,and always drop full red hearts!
*Can only be upgraded by cards & items for the Sewing Machine mod(ex.:Sewing Coupon(card),Sewing Box(Active),Doll's Tainted Head(Familiar),etc.)
- Dauntless Heart:
Soul Heart Behavior. When hit by an enemy, that enemy specifically takes 80 damage.
Alternatively this could be used for the Deceiver Heart as it has spikes on it.
- Deceiver Heart:
Doesn't show the spikes until you get close to it, similar to mimic chests.
Can always be picked up, and damages you by 1 heart on pickup, but grants a +0.25 damage up.
- Soiled Heart:
Soul Heart Behavior. When you enter an uncleared room, all enemies take a bit of poison damage (a-la Toxic Shock).
- Curdled Heart:
Rotten Heart Behavior. Spawns a clot upon clearing a room.
- Savage Heart:
Gold Heart Behavior. Upon losing it, you enter berserk mode.
I'm a bit unsure about this one, and it was suggested previously too.
- Baleful Heart:
Essentially just the Immortal Heart pickup from Antibirth. Fitting, too, as it is a The Lost unlock and it behaves like holy mantle.
- Harlot Heart:
Gold Heart Behavior. While you have it, acts as BFFs and Hive Mind.
- Miser Heart:
Gold Heart Behavior. While you have it, acts as a steam sale.
- Empty Heart:
Bone Heart Behavior. Instead of filling up with red hearts, however, picking up a red heart which would fill this up will instead give you a red locust for that floor.
- Zealot Heart:
Removes a heart container but grants 3 item wisps.

Slammable Cherries(Cherry Friends):Slamming cherries shoots 6 shots!
Crippling Slam & Fling(Cross of Chaos):When flinged black tentacles are summoned,these tentacles can kill enemies while crippling them. Slamming makes an area of tentacles appear.
Backfire Flail(Scalpel):3 tears are fired when Bertran hits an enemy with.
Rainbow Slam(Magic Pen):Instead of a double tap attack,Slamming makes a pool of creep
Static Slam(Angel Wings):Instead of double tapping,the attack is summoned when slamming
Piercing Head & Spectral Fling(Sinner's Heart):Flinging can make you wrap through obstacles & gaps! Hold “swap” to make your head deal damage through enemies.
Cursed Kissy Flail(Judas' Kiss):Hitting enemies with Bert’s Head will also inflict fear & bait.
The black chests on average seem to just, pay for themselves & more, I've never not regretted opening them since they repay with lots of hearts most of the time
The cards that just spawn 1-12 pickups are, crazy strong and require no thought in general
The starseer beggar is incredibly strong but I like him, I think maybe his costs or rarity should be increased, or have him spawn only if you hold planetarium items or some other pre-requeset
