It might be good to add MCM support so users can disable and/or adjust spawn rates for the new drops.
Also, how exactly did you get Scarlet chests? as a room clean reward?
Isaac- restock machines now spawn naturally in treasure/boss rooms
magdalene- bosses drop more health upon death
Cain- treasure rooms and shops doesn't require a key until chapter 2
Judas- either Devil/angel rooms chance is increased by 5% or all characters gain a permanent +0.5 damage boost
eve- razor now only does half a heart and gives damage for the floor/ more damage per use for the room
???- either poop is more common or grants more rewards like a passive golden petrified poop
azazel- black hearts deal more damage when depleted
lazarus- everyone now has +1 luck and +1 range
the lost- invincibility items last longer and immunity frames are increased
lilith- familiars are buffed to the amount of enemies in a room
keeper- +10 coins
forgotten- bone hearts now take half a heart of damage when no heart is in it
bethany- wisps buffed
j&e- small fading damage buff at the start of the floor
* Book of Virtues: Once per room, using an active item also spawns a second, random wisp.
* Lemegeton: The spawned item has a ~5% chance to be a quality 3 or higher Devil item, no matter where Lemegeton was used.
* Book of Secrets: Also removes all curses from the current floor.
* Book of Shadows: When the shield ends, Isaac pushes nearby enemies and projectiles away.
* Necronomicon: Passively grants the effects of Missing Page 1 and 2.

T. Isaac- reroll your life! unlocks more restock machines with different dice effects
T. Maggy- Overflowing love! Picking up a heart when you have full health or no heart containers gives a small fading damage buff.
T. Cain- Crafty! picking up items spawn 2-3 pickups on characters other than t. cain
T.eve- Stronger clots! Clots have more health and fade slower
T. Judas- just a little longer. fading dmg ups are stronger and fade slower
T.???- Holy crap! Ibs poops can sometimes replace regular poops.
T. Samson- unleash your rage! berserk and astral projection are activated for five seconds when you die once. Only activates once per run.
T. Lost- A gift from the Lord! Spawns a holy card at the end of every two chapters
T. Lazarus. perfectly balanced. choice pedestals are more common in all available rooms
T. Eden. madness unleashed! items rarely spawn in the wrong pool
T. lilith- Lost family... shops now have an intrinsic adoption papers effect. Regular shop items are sold on the left while one familiar is sold on the right
T. Azazel- expand your horizon. +1 range +0.5 shot speed
t. apollyon- mans best fly. Gives a locust to every character. Stacks with apollyons best friend trinket
T. forgotten. "Just a burning memory" secret, super, ultra secret rooms and vault rooms spawn a random item, picking it up removes everything from the room. akin to Death certificate. Picking up anything else destroys the pedestal
T. Bethany- A fiery spirit. Picking up items sometimes spawns an item wisp from any pool
T. Jacob- Neverending Legacy. Unlocks Soul of Esau, a 6 rooms active item that summons T. Jacob to aid you without damaging you, Curse of Esau, a curse which spawns Dark Esau that targets you. Tainted Jacob now spawns yet another Esau that stacks with birthright
Ceres - The starting room on every floor contains a low-quality passive item from👑or💀pools that cannot be rerolled.
Pallas - Isaac gains a spear that petrifies and damages enemies that touch it. Each enemy can only be petrified once. Every time an enemy dies while petrified the spear gets longer. The length resets every floor. (this one isn't very good imo)
Juno - All enemies spawned by other enemies are permanently charmed. Isaac’s familiars deal more dmg.
Vesta - A light blue flame follows close behind Isaac. It becomes smaller over time and when hit by an enemy projectile. Whenever a room clears it becomes bigger. If it goes out it reappears on the next floor. As long as the flame is alive Isaac gets an All Stats Up.
Chiron - Isaac becomes Immune to melee damage. Projectiles, lazers, floor hazards, and status effects still deal damage. Speed up.
You could also bring back a few of the cut repentance items like Donkey's Jawbone or Menorah if you're having trouble filling out all the unlock slots
Also Mark of Cain's effect doesn't seem very related to what it did in the Bible? You could make it smth like multiplying your dmg by 7 for a short while (because it was said that if someone harmed Cain, the damage would come back sevenfold). Otherwise, something that only works if you have familiars seems better suited for a trinket, if you ask me.
(Edit: I tried it, and Scarlet Chests definitely spawn way too often. Just through Crystal Key I got two entire items, which seems a little OP when the pool we're talking about includes Magic Mush & Brimstone)
If you don't have ideas for what the coffin should do, it could be a thing that only appears when Isaac has extra lives, and walking into it kills Isaac, then gives him stat ups relative to the health he lost.
Some of the "special" hearts also have weirdly high chances of appearing? Mainly Deserter & Benighted.
The following ideas are made under the assumption that those features haven't already been decided:
Dauntless heart could be like a sort of armor applying to a heart slot and making it so you can only take half a heart of damage on that slot
Soiled heart could be similar to rotten heart, spawning a friendly dip every time a room is cleared while it is held
Curdled heart could turn into a clot instead of healing Isaac when it is picked up
Savage heart could activate a beserk-like effect when fully depleted
Baleful heart could trigger a book of shadows effect when fully depleted (a sort of holy counterpart to black hearts)
All of them should in my opinion be found quite rarely
