And this can be especially difficult on real time MP servers where you can speed up time unless you sleep.
Well so far on my dedicated MP people are always surviving. And I am talking about new people to the game, constantly running here and there. I am more looking for some specific settings, because I am not sure if I will break something....
So I dont know what to tell you at this point.
The menu itself doesnt affect MP as far as I know, but maybe it does.
If anything try and crank the effects down so heavily on everything that it will make your players die no matter what, remember they have to be negative digits not positive digits.
I may guess you may increase period of infection-antibodies growth, so players have to manage their disease longer so more chances to fail treatment.
also, infection from bite should not start from 0 (as there are a lot of viruses going into a body with a bite), while antibodies shouldnt also....(player one of those immuned)
IRL it would be like 10:1 bite/antibodiese at start, and if player will be so lucky, it could at 7 day become 1:1,1

I wish your mod well, as you write it in description its very close to other vanilla mechanic, but I guess its need to be a little rougher and longer, because bites are no jokes...

Yes, that and the Resilient trait, which greatly increases your chances of survival. IMO gradual buildup of infection should be a thing otherwise you're forced into a glass-cannon build. On the other hand - each hit can be deadly so effectively the stakes are always high - no matter if you're facing a single zombie, a group or a horde.
