its RNG, l have gotten only 1 after scavenging the map a few times. but from what l know, search in "hidden" areas of the map or in the deadzone

It has a very rare chance to spawn at cult/ranger crates. You can open them at the NPC Safezone and you will get 4 high tier items.

I've also gotten two mystery boxes from airdrop loot. They did not start appearing until I had already unlocked the island and completed a few quests.
