This is supposed to protect you from dying while polymorphed (either normal or chaotic), and in all the testing and playing I've done, it always has protected me. However, occasional reports of deaths while polymorphed clearly indicate some sort of problem. The mechanism to protect you while you're polymorphed is especially hacky (because the way polymorph works is pretty hacky itself), so it's definitely not impossible that some circumstance is affecting people that I'm not accounting for.

So, is there anyone out there who commonly dies (not just once), when using this, while polymorphed?
Or at least, has had it happen more than once?
Do you use other mods (and if yes, which)?
Can you think of any unusual circumstances when you died while polymorphed?
Did you see a "Polymorph is garbage." message (which you should see when protected from a polymorph death)?

The ideal would be a reproducible scenario so I can get this improved.
