EN57-840, Electric Multiple Unit that took part in an incident under Żywiec on 19th December 2005. While operating passenger train 34527, driver Czesław Gołuszka noticed problems with proper stopping on stations. Moments later EN57-840 started losing its braking power completely, and after all attempts of bringing the unit to a halt failed, Gołuszka took the radio and shouted: “JELEŚNIA, DO SOMETHING, GIVE US PASSAGE, I LOST BRAKING”. Traffic dispatcher of Jeleśnia Station, Danuta Gancarz instantly jumped into action, giving train 34527 way straight through station. Several moments later, another train 43524, headed into opposite direction, operated by EN57-1271 and driven by driver Ryszard Budziak, received emergency radio notice from Gancarz: “Stop your train immediately, go to second cabin and run towards Żywiec, because train 34527 is heading straight at you, without brakes!”. 43524 stops and changes directions. Several moments later 34527 ploughed through Jeleśnia Station. Attempts to contact both trains failed. At one point voice of head conductor of 43524 – Maciej Polak – could’ve been heard in the radio, reporting visual contact with 34527, and then only providing distance between both units. After the last estimate distance being 50 meters, he retreated deeper into passenger space. Ryszard Budziak engaged emergency brakes and lowered both pantographs. Collision occurred when EN57-1271 was going approximately 70 km/h and EN57-840 about 20 km/h more. EN57-840 slightly derailed from the force of impact, both units now coupled were sliding down the rails for several hundred meters more, before finally coming to a halt.
Thankfully, due to the heroism of all people mentioned and not mentioned here, the tragedy was avoided, and no one lost their life in this only seemingly minor incident.

Manual:

Step 1. Before start
- Go to Cabin A, and turn On “Main Battery” and “Traction Motors Powerage”. You should hear an Alarm Buzzer go off, delete it with “CA/SHP”.

Step 2. Start
(In order to change from which end you control (Default Cabin A), switch flipswitch “Control” ON)
- Make sure “Control” flipswitch is ON!!
- Pulse “Safety Lock Disable”
- Raise pantograph A or B by pulsing assigned switches.
- Switch “Fast Cut Off” ON.
- Turn On “Converter” and pulse “Compressor”
- (Pressure in conduit will start automatically rising when pressure in main tank exceeds 0.3 MPa.)

Step 3. After start – prepare for route
- In order to gain "drive" ability, "Set Braking Force" should be at "-1" or "0". Main Conduit should fill automatically.
- Perform brake check procedure. Set braking force at values from "1" to "5", wait untill pressure in conduit drops and in cyllinder rises, then set braking force to "0", conduit should start filling in. You can speed up the process by pressing "Cylinger Pressure Release" button. Braking position "6" triggers emergency brake. To release pressure after emergency braking, set braking force at "-1" to perform impact filling of the conduit.
- (Optional) Choose D or R for forward drive or reverse drive (default D)
- (Optional) Turn on lights
- (Optional) Turn on radiotelephone and set channel, be sure to check radiotelephone 1-0-1!

Step 4. Driving.
- In order to drive, if on position 0 press "Position Up" once to go to position 1, or if on positions from 2 to 6, also move Drive adjuster wheel to position 1. Wait around 1-2 seconds until main circuit is closed. It is indicated by rise of amperage on "Traction Motors Amperage" gauges, and of course you'll notice because you'll start moving. Then player can add positions normally.
- Because of introduction of "Pressure Cut Off System" valve, and the "Linear Contactors Cut Out" everytime locomotive brakes are activated, by player or CA/SHP/Radiostop, or the Drive Adjuster Wheel is set back to 0, "drive" is gone, untill the Drive Adjuster Wheel is turned to position 1 for around 1-2 seconds, so the circuit can close.

Step 5. Multiple Unit Control System.
- Locomotive is featured with MU Control System (WORKS ONLY WITH UNITS OF SAME SERIES eg. EN57/EN71+EN57/EN71). In order to drive in this configuration, turn on main battery and traction motors powerage in both units. MU Control data is passed via Scharfenberg’s couplers at both ends of unit. No action other than coupling chosen units is required from user.
- Only startup of one locomotive is required, as the pulled unit will follow orders from the master locomotive.
- 'Master' status is not permanent. After turning off main unit it is possible to drive with the second locomotive as the leading unit, simply turn it on per standard procedure.
- REMEMBER TO DISABLE CA/SHP IN SECONDARY LOCOMOTIVE (with "Dead-Man Switch Disable" located in Cabin A)

WARNING! Resetting radiostop is individual for each unit with it equipped. Works only with units equipped with Radiotelephone.

I believe that you're talking about the edited paintblock acting as the CA/SHP indicator lamp. If by sticking out you mean it is not "hiding" itself into the front at all, try setting physics detail to low. If you mean it is slightly poking out like seen on the screenshots, don't worry it's perfectly fine, kind of necessary :)
