It's Lydia, she is the Housecarl you get in Dragonsreach when doing the main story past the first dragon.

I am not entirely sure what the issue would be then besides maybe a conflict in mods. do you have anything that modifies lydia in any way shape or form?

Thats most likely it. Have you disabled the manual armor management for her? If not she is going to wear whatever you have her set as.

i already had her as a follower along with others (i'm using Amazing Follower Tweaks) and does not seem to have changed appearance or armor could it be a problem with AFT?

I dont know I mean not very many people would complain getting a stealth based follower as a thane when they could go virtually anywhere else and get a tank/dps character.
