I disagree completely - two items gives so much more flexibility to mix and match with other items, for various looks.

I really think they should be one item, it's not really worth just the one item compared to alternatives available. If it was one item I'd be more inclined to wear it.

There is already an item set for Medic in-game which is called "The Mad Doktor", so I'd like to advice you to rethink your one's name.
