Report a translation problem
This isn't like Texas toast because why would you want a toaster on your head, it's funny I guess?. A fan is so generic that it doesn't look too out of place in hat when it comes to over the top war universes like tf2 or warhammer. Maybe Engineer needs cooling when working with heavy machinery or the hat is some sort of a computer that augments Engineer's brain so he needs cooling so he doesn't die from a heat stroke.
You want Engineer to suffer from Heatstroke?
In all seriousness this is creative, fitting to the game's artstyle, and works with a lot of outfits. When folks create something with a unique feature like this, they often feel the need to make something elaborate and wacky. Yet these more simple, practical designs are far more in-line with the Engineer, and just far nicer in general.
