Based Rome
There were some plans to institute a kind of puppet Byzantine Empire if the Ottomans were partitioned in the 18th Century (hence the existence of the ability to make a kind of 'puppet third rome' in this mod) but the whole 'Third Rome' thing was a reference to the city of the city of Moscow itself being equal in splendour and importance to Rome and Constantinople (Nova Roma, the 'second rome').
There was some theological stuff there about them being the only remnant of powerful Orthodoxy left, which I do try to lean into with the mod, but mostly it's historical nonsense given a different meaning by modern meme culture that I thought would make for excellent fodder for a fun playthrough, so threw it in there. :D
If you're after starting with a Hellenic character, you could use the "Harranian Immersion Pack" mod from cyberxkhan - it gives playable Hellenics in the Charlie and Old Gods starting dates.

Crack open the code if you're still confused - check the 'hre decisions' file.
Regardless of how you think about the HRE, it's the 'Rome in the West' because it already has a lot of unique mechanics in the game - legitimate to us, or not.
