From list there is nothing which should modify leaders/civs or religions as far as I can see. I do not have windows box to verify if this setup works on Windows. I also have noticed that after *first* start of the game after subscribing to Beliefs it works ok, but subsequent launches both loading a game and starting new one results in "one of your mods failed blah blah" as well as semi crash (semi as in, it doesn't technically crash but main menu buttons don't really do anything anymore, despite menu not being frozen).
If I disabled beliefs tweak only, then both FOREIGN_KEY error and belief_reliquaries error are gone: https://pastebin.com/6CT9WiGh and game starts normally.
