It's probably because Rimpy doesn't understand the contents of LoadFolders.xml correctly. The LoadFolders.xml description included in the mod corresponds to the *_steam packageId created by the RimWorld client under special conditions. It's just that Rimpy doesn't recognize it and doesn't handle it.
The description method of LoadFolders.xml follows the document published by Ludeon. Therefore, there is no problem with gameplay. Please report issues with Rimpy to the Rimpy team, not me. Thank you.

- If DE Surgeries, which is originally a prerequisite, is not enabled in the mod list and If you treated Dubs Bad Hygiene illness, the required medical skill for surgery is the default value of vanilla (12?). So added a patch to prevent it from being specified in default.
The after changed surgery difficulty was set to the same (medical skill level 8) as DE Surgeries (not HARD version). This patch verifies the existence of DE Surgeries and only performs the patching if the mod does not exist.
Check one. Do you have DE Surgeries HARD / HARD Color enabled in your mod list? Only these 2 mod have the required medical skills set to 12.
These two mods for high difficulty are set with the surgical difficulty and skill requirements used as prerequisites, so the conditions are increased by about x1.5 compared to normal DE Surgeries.
Diarrhea is a disease that can be cured even if left untreated, but the difficulty level is set to "normal" because it is not easy for doctors who do not have knowledge of medicines to treat it.
Therefore, the difficulty level is no "not unset", and the conditions are suitable for the type of DE Surgeries used. If you feel too expensive, consider using normal DE Surgeries instead of HARD.
