You can modify the config.cpp file in the South_asia_h.pbo and create a minimod that modifies the values in the City Names.
But people who make mods or create servers already know that...
it's a really big map, perfect for air missions.
And it can easily be played with a long distance map view (12km is very playable without low FPS).
I did the test with a wheeled vehicle driving at 100kmh and it took me from north to south only 9h of my life...
You have it inside the mod content... in the folder:
\steamapps\common\Arma 3\!Workshop\@South Asia\Missions\AirbaseTemplate.South_Asia_H
You can move this mission to your arma3 profile and merge it into your mission or use it as a template.

Yes this terrain was actually taken from Arizona imagery and not South Asia.
It seems that the BI gentlemen wanted to do something different initially or it was easier to get those satellite images in 2011.

This map is a development by Bohemia Interactive for the game "Take on Helicopters" within the ArmA universe.
It's really complicated to make something similar, I've only been able to adapt it to ArmA3 and improve it.
There are similar maps (for example those of PMC) for use in aeroplanes, but it's not easy to make.

inside the mod there is a mod folder there is a sample mission that has runways and lights. It's not very up to date, but maybe it can help you.

One question : are all airports with low texture runway/taxi way or is there any airport with "high quality" textures? :) Haven't found one yet!

It is very different to place them by hand in 3DEN and to place them in Terrain builder.
About river water, ArmA 3 doesn't support sloping rivers.
Impossible to add it with current tools.

It was tried, but the exact position of the building does not match in both mods, so just by changing the model path, the positioning is incorrect (exactly the CUP buildings are under the ground).
I have just checked it and it works correctly. I can see the runways and taxiways.
Maybe you are using some mod like "CUP Terrains Core" which is creating a conflict. I recommend you to use in such a case the small "ToH Dummy CUP" patch.

I have tried; Arma3 > !Workshop > @SouthAsia > Missions
I am using the AirbaseTemplate.South_Asia_H file and pasted it in missions on my arma 3 and still only get structures and buildings but no roads, taxiways or lights.
Inside the mod folder, you have a "Missions" folder where you have a template to use in your missions.
Just add it to your missions folder in your profile.

You must sort the mods in the right order as explained in the mod description...
Don't forget to use ToH Core.
I don't know if you use other mods?
Could you add a more extensive comment about the problem in the BI forum or by opening a discussion on Steam?
also this map is awesome looking cant wait to try its huge size
Perhaps it could also be caused by the latest update of @ToH Core v1.111, but it should not have a significant change.
