None of the introductory lines fire and I got the same result with another character.
Do any lines of the introductory conversation fire?
Can you try running the game with a new character and see what happens?
The bug you're describing sounds like a missing or corrupted file. Granted, I haven't played through it myself in about a year and one of the more recent updates might have broken something. If the vault copy doesn't work, I'll take another look at it.
Do you have any mods installed? Does the cutscene start, or are you just frozen in place? Did you try deleting the game files and reinstalling?
Hi, I had a glitch with not being able to interact with the ship anchor on the first shadow realm map as well - also managed to fix it after uninstalling Neverwinter Nights Facelift by Zwerkules (installed on Steam workshop).Haven't played through the rest of the module yet, hope there's no more weird conflicts.
Originally they were chained, but, since most of the invincible monsters don't have ranged attacks, it made things too easy for ranged PCs.
This would give a breather for PCs figuring out the riddle :)
The puzzle at the standing stones was tied to the items, not the creatures. IRL each of the humors is tied to one of the Greek elements: http://www.greekmedicine.net/b_p/Four_Humors.html
In retrospect it might have been a little too obscure.
My biggest criticism is the puzzle at the standing stones. I like the idea of using the altars to make the invincble creatures vulnerable. The problem is, even after getting past the puzzle, I still don't get the logic behind each artifact. For example, the Frost Banshee. At first I think I need to put the bile on the Water altar, since that's the closest there is to an ice element. Then I try the Fire altar, since that seems like the opposing element to the Frost Banshee. It turns out the Earth altar is the right choice, but why? As it stands now, you can't be sure you have the item on the correct altar until you land a hit on the banshee, and her cold aura can kill you very quickly.
The first disk is in the catacombs in the dead forest area. After you cleanse the catacombs, the disk is in the sarcophagus in the treasure room.
The second disk is in the catacombs in the dead village area. It's in the trapped sarcaphagus in the same room as the Shadovar Eldritch Knight.

If you replay the fight with the three of them, does he still keep resurrecting? I'm trying to figure out if it's something that happens every time you fight them, or just a one off deal.
Each of the four humours is associated with an element:
-Black Bile is associated with Earth.
-Yellow Bile is associated with Fire.
-Phlegm is associated with Water.
-Blood is associated with all the elements (but by process of elimination, Air in this case)
I just uploaded a patch that should address the issues you found.
Here are a few tips if you are struggling as a wizard/sorcerer:
-Use expertise mode when casting, the AC boost really helps with your survival
-Put Hassan in aggressive mode, and use the marking powder to get him to target specific enemies. Consider casting Flame Weapon and Haste on him as well.
-Improved Invisibility works really well.
-The damage shield spells (Elemental Shield and Acid Sheath) coupled with a DR spell (to protect your HP) can do a ton of damage to enemies.
-If all else fails, Bigby spells and missile storms are as effective as always.
I'll add a jump command to Rennard for his cutscenes. He should be following. If he isn't there, that will cause the cutscene to hang.
As for the Risen Helmites, I'll look at the script again. Hopefully I can have something by tomorrow.
