Redesign manual. Now fits on 5 pages instead of 11. (Also a few typo fixes.)

Fix Encrypted Equations in cases where one of the operands was None (100).

Remove option “ExcludeMinibossModules” since there are only two. Make it so “ExcludeIgnoredModules” doesn’t also ignore Souvenir since there’s a separate option for that.
