I am aware of two of the locations you are missing. Terramorphous Peak has a hidden vault symbol at the bottm of what looks like an elevator that can be found when walking away from the vending machines in that location. Also I know the location in Fink's Slaughterhouse which the symbol is location on a pillar near the corner of the arena that is opposite (diagonal to) the entrance of the arena.
