Meet up with the Biker.

Act 4: Scene 13 'Subway'

Making sure you don't skip the intro, pick up the phone just outside the room you start in, speak to Helmet from the first Hotline Miami.

Once you finish your conversation with him, continue through and complete the scene. After this, instead of progressing to the next scene, you visit a bar where you'll meet with Helmet and tells you about his past.

Once he finishes talking, you get the achievement. This is one of the three secret achievements in the game.

@Sienensiitaja It can't be real,as the other NPCs you see in there are mostly all playable characters.Pardo's throwing up in the toilet,the Fans are counting cash on the table bottom right,Beard is the barman,Jake is top left under Pardo and the Son is with the Henchman left to the table.
