I usualy play stength+enduarance build. M60 and FO-12 are just so strong. Also clubs are insanely effective and versitile (Chain mod or spike mod both are cool). I think, you underestimate the penetrator perk. Using this perk + AP ammo almost ignores armor at all (especially useful during horde nights against demos) and this perk is easy to up, especially if you go for lucky looter perk. Last 10 saves I played Insane with Nightmare speed and those skills are very powerful using that game settings. (sry for mistakes Eng isn`t my mothertongue) UPD And yes, you`re almost unkillable on this settings if equiped with raider set.

Stealth build is weak. Use only gigachad build with M60

For stealth builds, a lot of skills you list as bad are actually quite good: Hidden Strike, From The Shadows, and Archery are all good for stealth-dominant builds, allowing you to one-shot many enemies while sneaking without reprisal. Of course, if you get caught, you're running or hosed.

Many of the 'must-have' skills you list are across multiple attributes. Depending on how your playstyle goes, it may not be reasonable to get a lot of ranks into most of those until you have very high level (60+).
