The VPK for the model will be located in C:\Program Files (x86)\Steam\steamapps\workshop\content\546560\2911693161
Use GCFscape to open the VPK file and move the folders contained into C:\Program Files (x86)\Steam\steamapps\common\Half-Life Alyx\game\hlvr_addons\ADDONNAME
