His facial controls work, but as bones, not flexes/sliders. The rig_biped_simple IK rig works too!

EDIT: Bump maps work and it uses phong shading!
EDIT(6 Aug): His lips no longer stick together and I've cleaned up how his torso behaves when you rotate bip_spine_3.
EDIT(12 Aug): That last update made him smaller for some reason. His size is now normal again.

@SFXYZ #TF2Center yes, it should definitely be possible. Either start with the XNALara model, or this one (which you should be able to find in the SFM files, and then decompile with Crowbar), and then use Blender for the importing, fixing, and exporting. There should be tutorials online for each step

any idea how one could convert these to, say, a .fbx? Want to use this model for other things (namely rigging for use in the VR game bonelab).

I kust found https://garrysmods.org/download/1094/rins-dishonored-misc-items and https://garrysmods.org/download/1101/rins-dishonored-ports-ver-2 , but I dont know how to add them to something like gmod

There's a pack of Dishonored weapons and props which has been reuploaded to a lot of places after garrysmod.com disappeared.
