Ayo Cynic, thx a lot for this man.
I played this game like a year ago and as you can see on my Status on Steam, I'm playing it again. Missing 9 achievements that I've tried quite hardly to get but have not been able to.
Hopefully this, and your full guide may help on completing them.

Yep, you definitely can. Taking the new DLC route will save you having to fight Esdras, which if he causes you issues, will save you A LOT of time. The DLC route adds on an extra 5-10 minutes to the run which is good unless you think you can kill Esdras faster.

Either way, you do get quite a bit of padding on the time; I was able to do it in under an hour & a half.

Does anyone know if I can get the achievement Bronze medal and complete the game with True ending in a single playthrough? From what I understand, u need to go to the resting place of sister before u meet Esdras on the bridge in order to have the true ending.

Hey AniK9999, Not sure if I fully understand, but are you asking if you can unlock all those Achievements on NG+ or if it all needs to be done on a brand new Save? All these Achievements can be earned on NG+. Requiem Aeternam & Bronze Medal might be tricky to do in the same run though, but still possible. Hope that helps.

All 6 Health Upgrades
All 6 Fervor Upgrades
All 7 Relics
All 9 Mea Culpa Hearts
All 13 Prayers
All 15 Mea Culpa Abilities
All 6 Knots of Rosary Rope turned in to Soledad
All 30 (35 in my guide) Rosary Beads
All 44 Collectibles
Saved all 38 Children of Moonlight
All 8 (10 total) Vile Vessels filled
Ending A Achieved
100% Map Completion

As for Requiem Aeternam, none of the DLC bosses effect the Achievement & only the 10 Main Game bosses count. Unfortunately, Wounds of Eventide alters Esdras making him not a Boss which would void this Achievement for that playthrough.

How do I get 100% of the game, is it done by getting all the achievements or is it something else?
and another question
I have already defeated isidora, serpent and the final 3 bosses without flasks except for esdras :( because he went to the brotherhood of the mute lament and was the last one that I missed in that game of the true end.
Now I ask do I have to start a game again alone against the 10 bosses until the bad end to get the achievement?
