I tested it while playing as a 16 year old Sunni Egyptian, In almost ~30 years, I had only one child with my 4 wives. (played on the very easy difficulty too)
Anyway, if your wife is already pregnant for example, or even your female character as it was in my case, you can't just delete your unborn child... so be careful especially playing these kinds of characters with poligamy lifestyle.
As soon as you have one male, you can enable via the decisions. if your first child is a daughter, even better... free marry alliances.
