I had this problem too, I tried for weeks trying to find a solution, almost no documentation on how to solve it, and the documentation that I did find, didn't help at all, and things still wouldn't download. The mods didn't work or show up in my subscribed tab. I finally got in contact with this guy on discord, who had the same problem, and told me this solution, after I tried it thinking it wouldn't work, IT WORKED, so here is the soloution to hopefully make someones life SO much easier.

This issue occurs whenever your Mod.io token becomes invalid. You can resolve this issue by renewing your token.

1) Navigate to:

C:\Users%username%\AppData\Local\Insurgency\Saved\Config\WindowsClient.

2) Open the "GameUserSettings.ini" file.

3) Delete the bottom portion under [/script/modkit.modioclient] and save the file.

The next time you click on the Mods option in the in-game menu, you will have to re-accept the Mod.io terms of service. After that, a new token will be generated for you, which should fix the issue.

Just wanted to leave a comment for any wanderer that stumbles onto this solution that it still works but the ModIOClient script has moved from Engine.ini to GameUserSettings.ini (still in the same folder)
