Backup Files
Make sure you Backup any and all "ini" files. Do whatever you feel is best to backup files for yourself
Fallout 4 Config Tool Guide.
You can download Fallout 4 Configuration Tool if you wish. It comes with alot of other options that you may best disable and "optimize"

Fallout 4 Config Tool[www.nexusmods.com]

If downloaded follow the guide to locate your Fallout4.exe, I suggest replacing this Launcher as the default (You will be prompted if you wish for that).

Now Click on Settings

Now goto Performance and make sure VSync Mode is set to "0"

Then click save at the bottom Left.

If it doesn't allow you to save goto Settings and make sure "Warning for Invalidated Archives" is not highlighted.

Then repeat as above.

You may want to explore the other tabs and see what best suits your playstyle

You can disable controller input, change FOV, graphic settings and so much more....
Changing Ini files
If you don not wish to use Fallout 4 Config Tool then do this instead, if you have ALREADY done the Fallout 4 Config Tool guide skip this.

Goto you Documents\My Games\Fallout4 Folder and find the Fallout4Prefs.ini

Find "iPresentInterval=" and change the "1" to a "0"

Then save, thats you done for this part.

I don't suggest changing much if you don't know what your doing.
Nvidia Inspector
Download Nvidia Inspector[www.guru3d.com]

Run the program after downloading, make sure to extract the files into a folder.

Click on the Driver Version logo at the right

At the top in Profiles Find Fallout 4 (Just type it in the box to the right). In section 2 - Sync and Refresh you will have Frame Rate Limiter, click in that box and select your desired MAX FPS. Always better to go with whatever your Refresh Rate is of your Monitor. For mine it is 144hz, so I picked just under that at 143.3fps.

Then hit apply once you have done.
DONE!!
You should now be running Fallout 4 on whatever Native Resloution at your desired FPS without any Speedup of the game!

If you have any isssues, or want to add anything please let me know.

If I have mislead or mis-guided and not noticed please let me know!

Thank you, and I hope this helps you as much as it helped me!
UPDATE (July 2020)
Sorry for the lack of updates as I haven't been playing F4 that much recently, but decided to come back and play!

If you are noticing speech overlapping and just seeming not right and you don't wish to play the game like that, follow the steps below

Basically... Set the Max FPS Limit in Nvidia Inspector to 60FPS Limiter V2 (Picture Below)

That will remove the Speech problem and the game will still feel at 144 or whatever Refresh Rate your monitor is..

For me at 144Hz it feels like 144hz instead of 60.

You can also just download the Nexus mod to fix the physics and speech issues at high FPS,
This wasn’t available until recent, and makes this guide useless now, hope it helped anyways

https://www.nexusmods.com/fallout4/mods/44798

Any more issues please let me know!

And gives a wee Award on steam for the guide!

Cheers guys!

That works too! Does it fix speech overlapping

OR alternatively you can install the "Fallout 4 High FPS Physics Fix" from the nexus, uncap your framerate, and you're good.

Accidentally came across this. Are you saying that Fallout4 works at a 21:9 resolution with a simple ini tweak now? Before it required extensive modding and some things never did work right. Like scopes would never be round (but the cross hairs were still correct) and it would never rain on the sides of your power armor view and other minor things, but still annoying.

Yeah if you compare your sprint speed with iPresentInterval=1 vs 0, you'll notice it's definitely a lot faster than it should be. These types of guides will always be a minefield for engine issues.

Using something like Special K to force the game to run at a higher monitor refresh rate is the only way to get vsync/gsync to work with frame rates over 60. If you don't, and you only disable presentinterval, you're going to have screen tearing/microstutter and potentially (not really) physics issues. The main issue with both FO4 and Skyrim SE is they don't support refresh rates over 60hz. Disabling the built-in sync and going beyond that will make them run better but you're definitely not getting the real silky smooth 100+ fps you could see on a proper 144hz setting.

Thanks man, game feels much smoother now!

Sorry man, I can add it

Kovic - You are able to get 144fps because you have a monitor with a refresh rate of 145 (or a VRR of some other higher number).

What I meant is that if someone with a 60hz monitor tries this they're going to bork their game. Your guide doesn't clearly mention that so you're confusing a bunch of people who are unaware of that fact. I was telling you that you should clarify that.

ie "You must have a monitor capable of a higher refresh rate than 60hz to use this guide."
