Yes, there are 5 potions to reset each attribute individually, or 1 potion to reset them all. You get these from the HC dungeons or I believe from the electrum orbs too.

put "choose North America as the region", but most likely it doesn't matter. The graph remained from the vanilla version, when the servers were divided by region, like in Diablo III.
