Report a translation problem
-Each controller is connected via a separate "Microsoft Xbox Wireless Adapter for Windows 10" adapter (specifically this: https://www.amazon.com/gp/product/B00ZB7W4QU/ref=ppx_yo_dt_b_search_asin_title?ie=UTF8&psc=1)
-Doesn't ring a bell and I don't see anything similar in appwiz.cpl. Controllers are current (updated via the Xbox Accessories UWP app).
-Restarting device inputs works until PC is restarted. Closing & reopening Steam retains previous configuration.
Also important to note that the exact same non-Steam games launched OUTSIDE of Steam do not have this issue. Only occurs when added as a non-Steam game and launched from the client. Tested with the Yuzu emulator, Elder Scrolls Online (retail, not Steam version) and a couple other small games, same result on each.

I assume this would be a problem in Steam games as well, if it's showing up in the Steam controller test UI.
The Xbox Extended Feature Support Driver is available in the Steam controller settings, if you scroll down.
-EDIT: This is correct, not sure how I didn't notice it before. Tested with 2 native Steam games (Control Ultimate Edition and Octopath Traveler). For both though, the RT-held-down bug seems to have only occurred after pulling RT once, whereas with non-Steam games it launches that way.
-Located the Xbox Extended Feature Support as described - it was already installed prior to all noted testing. I must've done it during initial controller setup and forgotten, apologies.
It is 100% replicatable for me, but my setup is unique. My gaming PC is in a central location in my house and I use it from multiple different rooms, each with separate controller/kb/m/displays. The reason this is relevant is that the Xbox Wireless Adapters don't behave well if multiple are plugged in simultaneously - controllers will lose their pairing or refuse to connect altogether, things like that. To combat this, if an adapter is unplugged and plugged back in (or in my case, dc/rc with a toggle switch), the controller will pair again normally. I don't know this is contributing to the issue, but it's possible considering you were unable to replicate.
I realize this is a very niche issue and has a functional workaround so obviously very low priority as far as bugs go. Just wanted to report it since I found that workaround, in case anyone else comes across this seemingly-obscure problem and is struggling to find a fix.
