I'd question WHY you want to but then again I *have* done a murder-spree Dishonored run before, so... I mean. I know the thrill. But don't expect to get a 'good' ending...

Semi sure about Baldur's Gate and other Infinity Engine games, quite sure about Planescape: Torment. Although I think you might run into troubles with the god-like entity residing over the city there.
But I think many of the older RPGs allowed for complete killing of the populace.
Not only kill, but they also could BE killed. Something I really hated about Skyrim when they just kneel down while enemies are flailing away at them. Compared to Daggerfall/Morrowing the series adapted so many immersion breaking changes, it's just sad.
Any games like "Yandere Simulator" in which you can unalive anyone just the way you want to?
