What’s the biggest plot twist in a game?
For me, it was Frank Fontaine’s identity reveal in BioShock. It was such a great build up throughout the game.

Sovereign's true nature in Mass Effect.

The original game arguably qualifies as hard sci-fi, with a well-grounded, scientifically accurate universe. It makes it all the more disturbing when the true antagonist is revealed as basically a Great Old One from the Lovecraft Mythos, whom the apparent big bad is absolutely terrified of.
Final Fantasy X. Your dad was actually the main villain the whole time.

definitely a good twist, and there are being some subtle hints and foreshadowing of this too long before it is revealed that sin is jecht

i think he truly was crippled at first but attuning with the axiom in apocalypse gave him the ability to walk again, among other supernatural things.
For me, probably:

  • Revan being the player in KOTOR
  • John dying in RDR - not really a 'twist,' but I didn't see it coming. And it was amplified by the futility and tragedy of his son ending up making similar choices after he was gone in the playable epilogue.
  • The Flood in Halo CE. The entire premise, vibe, and feeling of the game completely changed at that point. Incorporated mystery, some horror, cosmic stakes, etc. Did not expect that back in the day while playing for the first time.
  • Realizing what James actually did and what was happening in Silent Hill 2.
  • Shodan being the antagonist in System Shock. I legitimately never saw that coming back then, which made the whole experience a lot more compelling than I think it might have otherwise been.
  • The Dark World being a thing in ALttP. Such a cool surprise.
  • Teo's death and Gremio's return/survival in Suikoden. For such visually simple, text-based JRPGs, those games could really be emotionally affecting, and I didn't see either of those things coming.

Alan Wake; His wife secretly tries to get him committed in a clinic, instead of taking him on a vacation.
But AW has a very well written story that is full of plot twists, and people should totally play it if they're into horror stories. It's an absolute classic.

Actually, yeah let me add that to my list as well. Also, Jade Empire is one of my favorite BioWare games and feels criminally underrated to me just because people felt like it was "just KOTOR with martial arts." Which... yeah it was. But the thing that made it cool was... well... it was KOTOR with martial arts lol.

Probably more than half the game but there are some early hints that something is amiss like Ameena looking like Sophia (almost too obvious here) and the data manipulation/technology lore of their universe.

Nothing tops Silent Hill 2.
These are some that stood out to me...

The Legend of Zelda: Ocarina of Time -- Using the alter at the Temple of Time for the very first time
Final Fantasy 7 -- Aeris dying
Legend of Dragoon -- Lavitz dying
RE: Code Veronica X -- Alfred going stealth as Alexia
Star Wars KotOR -- Revan's identity reveal
Lost Odyssey -- finding out Kaim's daughter is actually alive(only for her to die soon after), Gongora's backstory
