Strange Issue with my GamePad Xbox 360 controller

Hi all,
I'm having a strange issue with ym GamePad Xbox 360 controller I'm using on PC to play Fallout 4. For some reason when I hit LB (to use VATS) it opens the screen magnifier and then when I hit the start button it minimizes to the desktop. This issue just started today and I am having trouble resolving it. This is what I have tried so far:

1.) I tried turning the magnifier off in the ease of access settings. Even though the magnifier is off (and yes I restarted my PC after doing it) it still does it after about 15 minutes of game play.
2.) I tried changing the controller settings back to default. I had a bit more success with this, however after nearly two hours of game play it started doing it again.

I'm not sure whether the issue is with the controller, the PC, the game, or what so if anybody has any suggestions or outright fixes that would be awesome. Thanks!

Switch to Steam Client > Settings > Controller > Desktop Config > Disable
