No, they don't.
"However, do note that games which have been played or keys which have been revealed/redeemed are likely ineligible for a refund, as are Humble Monthly bundles."
An authorized 3rd party seller can not revoke a key, so they do not offer refunds on game keys.
As they have never been able to revoke keys, that is unlikely. It may have been for a key you did not reveal, as per the support article. I've used HB since the begining and that policy has always been there in reguards to no refund of keys.
Otherwise people would have simply scammed them for the keys, then requested a refund.
They refunded and removed the key from steam.
Thomas seemed fully willing to do a full refund for me but also noted that they did not have the ability to actually remove the key. I didn't think that seemed right (or fair), so I instead offered the idea of a partial refund (to basically price match the new sale price) since then that at least addresses the whole issue of having been screwed by false advertising to pay a higher price for the game they said I could play on day 1. At least with the partial refund, it'd be as though they were more up front and honest about it and given the information that would have made me choose to wait until the Mac version came out.
Now, this was a pretty extreme case and I did decline the full refund and instead opted to take a partial refund instead since the key couldn't be removed. So you're right. It takes an unlikely case for Humble to issue a refund.
That said, I've also seen other third party stores who outright ignore their own refund policy *cough*Gamersgate*cough* and deny it even when I qualify (Wasn't a Steam game at all, wasn't downloaded, was within their "no questions asked" return period. Still denied with no reason given). So it's something that depends on where you got it and why you want a refund.
