Keep in mind that Adult Only Sexual Content games category is disabled by default. So, don't complain about seeing such games after enabling seeing such games.
You may have forgotten to turn it off after playing with settings, or you may have did it by mistake. Otherwise it wouldn't be on to begin with unless you wanted to see adult content, or you somehow convince yourself under some misunderstanding what it does thinking to find GTA, witcher, or etc which those are not adult games even if you think they are they are not label as such.
Do what post one said, turn it off, and your problem is solved that it. If want gore games to show up, enable gore that it, but you do not need adult content to be enable.
You do know that kids also can see the goregames?
It is not really that hard to put in numbers to tell the system that you are old enough....
You might have accidentally ticked it at one point. People do that, sometimes unintentionally or even intentionally because they wrongly think that the AO filter also filters games like Witcher 3.

One, however, is more socially acceptable for a movie and/or game then the other.
Or it depends on the country you are from.

Most people in most countries would have said ♥♥♥♥ happens, move on.
The US came to the conclusion that they need to broadcast live events time shifted so that they can switch the camera in case something like that happens again......

Both are dependent on context.
Someone being tortured and dismembered in a movie is acceptable. Someone doing so in real life is not.
No afraid, but it is not considered acceptable for people under a specific age. Running around naked for no reason is also not acceptable.
There are countries where showing an ankle will get one arrested, let alone nudity.
You may want to check on what other countries laws are, as you may fine the US isn't really a good example.

The FCC doesn't allow nudity to be broadcast, especially for events that are considered PG13, though there are exceptions and requirements that it can be allowed under. The time shift was needed to stay compliant with the law, in case something like that (or similar) happened again.
I'm keeping it enabled because I saw extremely silly things like Sex Chess or Sex with Hitler, but I know I'd never go out of my way to enable it myself
I kinda wish there was a toggle for age gates in general, but meh, that's still not gonna happen any time soon.
