^ This happens upon changing screen res, but not always (it rarely happened to me)

There are alternatives, since id Software has released the source (like ioRtCW)

Edit:
Well you can see my guide where I tried to sum up most common issues and their work around :P

Go to \main and edit wolfconfig.cfg with textpad and edit these lines:

seta r_mode "x"
Set this to -1.

seta r_customwidth "1026"
seta r_customheight "768"

and save.

This worked for me ;)

Find wolfconfig.cfg in your game folder and alter the following
to the same values , where xxxx is your resolution

seta r_customheight "xxxx"
seta r_customwidth "xxxx"
seta r_mode "-1"

You can reset the config file by removing it, the game will automatically regenerate it with the default settings

Just download the RTCW 1.42c patch. It should resolve all your problems just extract and overwrite them in your steam folder. Check google for the download link

Sounds like your choosing a resolution your monitor cant do.

I have the same problem, except it's my first time starting the game and I haven't changed any settings yet.
Windows 8 64 bit.

I had the same problem and I fixed it, delete the "gl" folder, it worked for me.
I will update if I have any problems.

You put it where exactly?

I guess with the EXE files, usually under Windows the binaries look for DLLs either in system32 or their own path
