Comes from a bug in the original Civilization, where Gandhi was so peaceful he could suddenly become extremely aggressive. Players liked it, so Sid Meier kept it in the succeeding versions of Civ.
-They believe your military is that much weaker than theirs.
-You have touching borders.
-You've been doing anything that they don't like (examples include, "you're winning").
-Any combination of the above.
Plus, as mentioned above, Gandhi is also a special case.
In the first civ the game doesn't know how to count below 0. Gandhi was set to not only be almost 0 in warmongering, but also default to taking Democracy which for the AI lowered their warmongering even lower.
However in Gandhi's case it lowered it so much it wanted to go just below 0, the game didn't know how so reset the chance from the top, suddenly making him the single most warmongering civ on the planet.
And so, a legend (and running joke) was born.
Gandhi is very peaceful and friendly. But he also has the highest tendency to pursue and use nukes (due to a bug in the first game that was kept as a running joke).
