Watch Dogs 2 got flak because if you shot and murdered a prostitute and inspected her corpse, you could see she wore no underwear under her skirt. They had to change that. Now you can murder innocents freely without having a chance to see something 50% of the world has.
I have seen youtube vids of The Frest where the creators had to censor the breasts and vaginas on a gruesome mutant monster until they were covered in enough blood so that wasn't visible anymore. A same thing happened with a horribly mutilated corpse in a Hannibal tv series (iirc) where the exposed butt had to be covered in blood to get greenlighted.
There are countless examples of this backwards ♥♥♥♥♥♥♥♥, mostly in the US. And as long as that exists we'll keep having this stupidity of NPC's showering in underwear.
edit: Jesus, even Steam joins in on this prudishness by censoring the 'V-word'. I bet I can say rape, but I can't say vagina. Edit2: pleasantly surprised. Still stupid though.
Nudity in the US is not frowned on near as badly as it was in the past. You can look at several games released in the not so distant past that had nudity.

It's gotten a tiny bit better over time but it's still nowhere near close enough for 'full frontal nudity in showers' you're asking for.
I believe all those titles you mention are 18+ and either play it safe by only showing breasts and butts, or are Conan and South Park who really don't give a ♥♥♥♥.
It's not about the rating (so much anymore) it's about the content. Many physical outlets will *not* buy a product that encourages sexuality in specific mediums (stuff they don't like, but tolerate as it gets them business)
The list you had is kinda perfect (except for South Park/Heavy Rain, which did not have a nudity rating in it for years)
Publishers and licencers will not encourage a product that may scare off potential chain/resellers. The Witcher series had a lot of mall-bans. Duke Nukem had plenty, God of War had more.. GTA has record-number bans..
It really doesn't matter online, as nudity is (le gasp) more welcome when you're making a purchase without the eyes of someone who can judge you.. but this is one of the biggest issues that physical copy sellers have. Game Of Thrones has a really interesting part in it's interview about wanting an implied-nude model on the box art, but was denied by HBO, because of potential ban scares. (despite HBO being very interesting in encouraging sexualization/pornography in their material.. and Game Of Thones having it in spades)
Anyways. While it wouldn't effect many sales on the average, everyday gamer.. This wouldn't encourage sales either when you also have random impluse buys for physical copies on the line..
You can't have your cake and eat it, here. Maybe if IO decided for an online release only? But they've not been successful enough for that.
Almost nothing of Diana is shown during that scene except a glimpse of her backside and even then it's through steam and blurry
