Where did isshin get that gun?
Did I miss the part in the cinematic opening where isshin just capped the opposing general with his semi-auto pistol? Why is he the only person in the game with rapid fire pistol? For a sword saint his two most deadliest weapons are his spear and revolver or whatever kind of pistol that he's got there.

Likely the same place he retrieved Gyoubu's spear, that being the underworld or something like that.

doesn't explain why no soldier in the entire game has a gun anywhere close as good as his. Not even a miniboss with a gun exists.

It does, though. I'd call it an ex-machina. No other character in the game is resurrected from the black mortal blade sacrifice; no other character returns from the underworld. Isshin's gun is otherworldly and therefore doesn't totally need to abide by the rules the game's lore set thus far.
This is also irrespective of the other immortal beings, who are described as "infested", that having to do with the centipedes.

I have nothing to you on the gun front other than he got it from the same place that one dude in Katanagatari got his double semi-auto pistols from... meaning a mad dude literally looked into the future and copied their designs.

But regarding the "Sword Saint" title, that's the translation for the title of “Kensei”, which was given (most commonly post-mortem) to legendary warriors who mastered the arts of combat in any form (in real life Japan I mean). The term does have the word "ken" in it, which typically refers to swords but is generally used for blades and weapons as well, not just swords or your typical uchigatana. So that's that bit of the "mystery" explained.

To be fair, Miyamoto Musashi (one of the real-life "sword saints" and the guy Isshin is possibly based on) liked to play dirty too, because the duels were usually to death and being alive is better than being dead. Funnily enough, he dual wielded too (not with a spear though, but an off-hand short sword), even founded a school teaching this style.

There is a place called the gun fort... not hard to imagine Ishin had something specially made for him.
