They do their regular attacks when faced against creatures of their same size or larger. As for swinging their giant arms, you can consider that a part of trampling. It's no longer really a targeted attack but a sweep that clears several smaller enemies at once!
Also note, from page 50 of the Manual, that trampling is an armor-piercing attack *and* does a minimum of 1 Damage, so it can be useful against Glamoured/Mirror-Illusioned foes.
Presuming equal attack, size6 trample will do more damage against a single target of 23 or more protection. Prior to that, the halberd will either do more damage, or be tied but its slashing +25% mod and dismembering coming on top. Against targets with slash-resist the trample is always better.
However, the trampling creature can often hit more than one target, three being the most likely density into a trampled square. Meaning it actually does damage x3. And this is not to say the trampler will hit just one square in the time a ghoul swings once, could be more.
