Second Act companion quest not triggering

As the title says, I cannot trigger the last part(the one requring the item) of any companion quest. I can no longer level up as there is nothing left to do in the act, and I am currently stuck as 5druid/8shifter with over 7k exp left to the next. Tried using the console to give myself the xp but it didnt fix anything.

I started companion quest as soon as it was available, and spoke to Grimgnaw after every level up, yet for over 2 levels nothing changed. Also, the same thing happened with the druid circle challenge. I defeated the first opponent but couldn't fight the next.

At first i thought it was because i shapeshifting but it didnt matter. Is it possible the game considers the level of the shape i was in at the time of the conversation as the level i am supposed to be?

This only happened to me when I skipped Act 1 and immediately started Act 2

Did you do the henchmen quest in Act 1? You must do the Henchmen quest in Act 1 to get it in Act 2
