its sometimes buggy and everyone wants to help so there are more people want to help than that there people searching.
And there's probably a 50% chance i end up either in the final boss, the duo or in the cathedral but those are probably just where people want support most.
If you or whoever the host is you're connecting to has to many bloodcode loadouts, it'll cause that problem.
Buurt, if you're attempting to farm for Marks of Honor, it's best to do it in the Depths.

exactly, thats how i farm my medals, quick and easy 3-4 medals each summon
The only real distinction is, using the signal in the normal world and in the depths.
But in both those you can end up anywhere in the world, or anywhere in any of the depths when awnsering a distress signal, likewise when you send a distress signal, any player anywhere in the normal world or in any of the depths can recieve it.
There might be some hidden conditions like level or if you have already beaten a boss before you can be summoned, but I'm not sure about that.
Never tried multiplayer in the early parts of the game when I was still low level.
That's probably to prevent spoilers, or to avoid people fighting bosses they don't know how to fight yet.
I noticed this when I realized I can't connect to a friend who is just starting out (who's on the sewer boss) even though I'm on new game plus (Oliver), but he can connect to me. The moment we've finished doing sewer boss on my playthrough (now we have the same progress), I can now finally connect to him.
Now I'm staying in the endgame (Just before Silva) without going NG+++, and I'm staying there while looking for people to assist. So far I'm able to assist people from any part of the story that way. I've noticed that lots of people are asking for help with Emily, Juzo Mido, Gilded Warrior and IceFire Tagteam, I'm able to connect to all of them just fine.
Some are a bit laggy though, even with the country restriction. But still very much playable.
May I suggest that the devs add a note in the multiplayer screen clarifying these criterias in the next patch, or make it so that NG+ players can connect to anyone, they've already beaten the game after all, so they'd know what to do.
Edit: Though to assist in the depths, you have to be on that same map to receive signals for it.
Progression-wise, I'm at Silva's bossfight (in fact im NG ready, the blinking star is on my homebase door), but I'm searching for signals on the Oliver area, or sometimes at the ribcage boss area (Aurora is dead in my current playthrough, so her arena is rather serene) and I'm still able to connect to other players.
