Question three and five - there isn't really a world changing quest imo. And for question five, only Xu can come confessing to your character. Sadly he does not propose. Only ones who can are just Gust and Mint only if you give them the knot first in order to trigger the event, which kind of suck. I really hope they consider other characters come approach your character in the future.
Question four - If this is for Phyllis, you cannot help achieve Phyllis's dream unless you marry her.
3- it kinda is world changing if you are setting up a new clinic which leads into......
4- shame coz this effectively locks you out of content that shapes the game world unless you marry someone :(
5- hope this gets expanded upon then i've never seen a game that lets npcs ask YOU to date before it seemed like a novel idea.

I now have a different question though coz for the life of me i can figure it out, i gave phyllis the knot thing and we became soulmates but there's no option or item i can find to get from soulmate to lover in order to marry her, i try to give her another knot but she says 'i don't need a second one'.
How the hell do i get to second base? feel free to joke about that question lol.
Just a warning, in case you have 14 soulmates, like me:
When you marry, it will knock every lover/soulmate down to three stars/non-lover (from max). You may have guessed but I wanted to mention, in case it helps. Good luck all!
In all seriousness though that must have been gut wrenching when you suddenly lost all that gifting progress thnx for the warning.
When I did this, there was no jealousy yet in game (they were all maxxed before that). I haven't lost the relationship progress because I never got married in a saved game: I kept all the lover benefits. Having no helper is a bit rough, so I was very happy when Ack got fixed ;)
I'd really like to experience the whole thing though, and will start fresh with new toon on day one of full release :)
